"Our ACO model focuses on the most vulnerable, highest acuity, and under-supported Medicare population by bringing together clinicians, facilities, data, and technology to drive quality.  When we lead with quality, patient care will improve."

Mike Camacho
President, Sound Long-Term Care Management

"At our core, Sound is dedicated to providing quality care across the patient population on a national scale. While other ACOs act as an administrator to their participants, or simply a data-sharing entity, Sound is a committed and active partner.  In addition to full transparency on reporting and data, we bring the backing of all our clinical services lines to mitigate risk, offer support and ultimately, work as a team to increase the quality of care these patients deserve."

John Dickey, MD
Chief Medical Officer, Sound Long-Term Care Management

Innovation

Sound's core value of innovation was a guiding force in developing the SLTCM care management model. Leveraging two decades of Sound's clinical performance process, the model will help hundred of physicians and their facility partners achieve quality metrics with education, resources, workflows, technology, and improved care coordination.  And Sound's near-real-time data reports for participating physicians will help adjust course and drive behavior changes to align towards consistently better patient care.
