SLTCM is a Medicare Shared Savings Program (MSSP) Enhanced Track ACO and concentrates on caring for Medicare beneficiaries who are residents of assisted living and of long-term care facilities - one of the most underserved populations in the U.S. within ACOs.

SLTCM is lead by Mike Camacho, President, and Thomas Kim, MD, Chief Medical Officer. Both Camacho and Kim have deep roots in long-term care and value-based care. Camacho has spent the past 15 years working on ACOs, MSSPs, and clinically integrated networks, and Dr. Kim brings more than a decade of clinical leadership and expertise in value-based care, with unique insights in episodic care and post-acute and acute care models.
